Job summary

VOALA is seeking a Case Manager Specialist I to provide intensive case management to eligible clients, meeting participants weekly. You will be the primary contact to coordinate services and care for assigned clients, handling a caseload of individuals needing a wide range of human services resources.

The role includes conducting screenings and assessments, linking services, care coordination, housing navigation, and advocacy.

Qualifications

• Must pass fingerprint clearance and background checks.
• Valid California Driver’s license and reliable vehicle for home visits.
• Ability to perform under possible emotional stress when working with vulnerable clients.

Responsibilities

• Conducts comprehensive screenings and assessments to develop a case plan.
• Provide service linkages and support systems to meet identified needs.
• Care Coordination and Case Management of assigned caseload; housing navigation.
• Coordinate individualized planning with clients to meet short- and long-term needs.
• Advocate for participants before agencies and community bodies.
• Must drive to client meetings and home visits on a regular basis.

Education

Associate’s Degree in Social Work or related field

Two years’ total education/experience (case management background)
