Job summary

VOALA is seeking a dedicated Case Manager Generalist to provide on-site case management services for low acuity clients. You will coordinate assessments, develop action plans, and connect clients with housing, employment, and health resources to support their life goals.

You will work closely with a multidisciplinary team, conduct screenings, and advocate for participants with community and social service partners to ensure comprehensive care. This role is on-site in the Los Angeles area.

Qualifications

• Fingerprint clearance and background check are required.
• HS/GED and VOALA training within 6 months of hire.
• 1 year of general case management or a related social services field, or a BA/BS in a related field.

Responsibilities

• Conduct comprehensive screenings and assessments to collect functional, environmental, psycho-social, financial, employment, housing, education, and health information.
• Coordinate case plans, services, and advocacy to meet short- and long-term client needs.
• Advocate for clients before agencies and service providers; coordinate care with the team.
