Qualifications

• Fingerprint clearance and background checks required.
• Ability to work under emotionally challenging conditions.
• Completion of VOALA Case Management training within 6 months of hire.

Responsibilities

• Conduct screenings and assessments to gather relevant client data.
• Develop and coordinate individual case plans and services.
• Link clients to resources and supports; monitor progress.
• Advocate for clients with agencies and service providers.
• Collaborate with team to coordinate care and community resources.
