Job summary

West River Mental Health in Rapid City, SD is seeking a Case Manager for our Full Circle program to support clients in recovery during and after treatment. You will lead life skills groups, conduct intake sessions, and develop person-centered care plans in collaboration with clients and staff.

You will track progress, connect clients with community resources, ensure HIPAA compliance, and participate in team meetings while maintaining accurate documentation and a flexible schedule that supports

Qualifications

• Minimum high school diploma; human services experience preferred.
• Willingness to address substance use issues and support recovery.
• Ability to collaborate with clients, families, and community partners.
• Strong organizational skills and attention to documentation.

Responsibilities

• Lead life skills groups to build client daily living skills.
• Conduct intake sessions to identify client needs and goals.
• Develop and implement person-centered case management plans.
• Track progress, educate on community resources, and plan aftercare.
• Locate support groups for clients and families.
• Coordinate with community resources for holistic care.
• Maintain safe recovery-focused environment and complete documentation.
• Attend team meetings and training for professional growth.
