Job summary

HealthRIGHT 360 is hiring a Case Manager to coordinate intake, assessments, and benefits across outpatient and residential services in the San Diego area. You will work within a multidisciplinary team to connect clients with mental health, substance use treatment, housing, and employment supports.

Responsibilities include completing intake paperwork, assisting with resume writing, scheduling appointments, and ensuring timely documentation while meeting productivity expectations.

Qualifications

• High school diploma or equivalent.
• Substance Use Disorder Counselor registration from an accredited CA agency (CCAPP, CAADE, CADTP).
• At least two years of experience as a job developer or employment specialist.
• Experience teaching adults and/or facilitating workshops.
• Commitment to working with diverse communities.

Responsibilities

• Assist with intake by completing case management assessment and entering financial and benefit information.
• Support the client in applying for Medi-Cal or transferring Medi-Cal County when appropriate.
• Coordinate communication with external service linkage including appointments, probation, DCFS, housing, and employment services.
• Transport clients as needed and complete client UA tests and document results.
• Prepare clients for job search by resume writing and interview preparation.
• Maintain timely progress notes and compliance with documentation timelines.
• Attend supervision and team meetings and participate in trainings.
