Job summary

COMMUNITY CARE OF WEST VIRGINIA INC. seeks a full-time case manager to oversee caseloads of adolescents and adults with long-term mental illness and/or addiction. You will assess needs, develop treatment plans, coordinate services, and document progress in the patient record.

Responsibilities include face-to-face reviews, referrals, and collaboration with psychiatrists and staff to ensure compliant, effective treatment. A healthcare background and strong interpersonal skills are essential.

Qualifications

• Combination of behavioral health work experience or related education.
• Bachelor’s degree in social work, psychology, or human services required.
• Two years in a healthcare setting, preferably behavioral health.

Responsibilities

• Manage an assigned caseload of adolescent and adult patients with mental illness or addiction.
• Assess needs, develop and implement treatment plans, coordinate services.
• Maintain case records per regulatory standards and requirements.
• Facilitate referrals to community resources and external specialists.
• Communicate with behavioral health staff about patient status and follow-up.
