Telecare’s Recovery Starship Casa D in Riverside seeks a full-time case manager serving adults with serious mental illness. Under supervision, you help clients live independently, coordinate Recovery Plan services, and work with a multidisciplinary team to deliver supports and transportation as needed.

The role requires strong communication, cultural sensitivity, and navigation of community resources, with a focus on safety and confidentiality.

Formación

• Required: High School/GED or higher with direct mental health/community services experience
• CPR, CPI, and First Aid certifications or ability to obtain within 60 days
• Must be 18+ and have valid driver's license
• Post-offer checks include background, licensing, and medical clearances

Responsabilidades

• Serve as primary support and case manager for individuals with mental illness
• Collaborate with team to create Recovery Plan goals
• Ensure Recovery Plan services are provided
• Coordinate transitions to lower levels of care
• Maintain confidential records and HIPAA compliance
• Provide transportation and linkage to community resources
