Job summary

Restorative Pathways is hiring a full-time Case Manager for Riley Recovery Centers in Oakland, CA. This hybrid role coordinates case management across sober living and clinical programs, documents activities, and supports discharge planning.

You will work with the Program Manager, AOD Counselors, House Manager, and clinical team to ensure seamless client care, housing, and transportation coordination while maintaining confidentiality and HIPAA compliance.

Qualifications

• High school diploma or equivalent.
• Ability to pass all required background screening.

Responsibilities

• Serve as primary point of contact for case management needs throughout residents' stay.
• Coordinate collateral contacts, referrals, and appointment coordination for PHP and IOP.
