Job summary

Promises Behavioral Health is seeking a full-time Case Manager to coordinate comprehensive client services during residency and post-discharge planning. You will deliver psychoeducational and motivational sessions, document in medical records, and collaborate with the program team to ensure quality care.

The role requires strong clinical skills, meticulous documentation, and the ability to manage multiple priorities in a fast-paced environment.

Qualifications

• Knowledge of and experience with case management.
• Experience with substance abuse, mental health, housing, and community support services.
• Experience building rapport with substance-abusing populations and providing referrals.

Responsibilities

• Screen, intake, orientation, and assessment of patients.
• Develop and refine individualized service plans; arrange referrals and transportation.
• Provide at least three individual case management sessions per patient per week and document reviews.
