Job summary

HealthRIGHT 360 is seeking a Case Manager I to coordinate care across outpatient, residential, and community settings. You will support intake, benefits applications, and treatment plan implementation for clients with substance use disorders.

Reliable transportation is required to facilitate services across various locations. You will collaborate with clinicians, deliver documentation in a timely manner, and assist clients with job search and aftercare planning.

Qualifications

• High school diploma or equivalent; SUD Counselor registration from a CA agency (CCAPP/CAADE/CADTP).
• Bachelor’s degree in Social Work, Psychology, or related field; SUD Counselor registration from a CA agency (CCAPP/CAADE/CADTP).
• Case management experience.
• Experience with clients in substance use treatment and relapse prevention.
• Valid California Driver’s License and access to transportation.

Responsibilities

• Assist with intake by completing case management assessment and entering financial/benefit information.
• Support client in Medi-Cal applications or transfers.
• Assess client needs and manage releases of information.
• Connect clients to benefits, health services, housing, employment, and community resources.
• Coordinate communication with probation, DCFS, and court-related requirements.
• Deliver services in outpatient, residential, or field settings as needed.
• Prepare clients for job search and resume development.
• Monitor progress and provide input to treatment plans.
• Collaborate with treatment team for comprehensive care.
• Transport clients as required and perform UA testing.
