The role emphasizes intake, counseling, and aftercare within a strengths-based framework to empower clients toward independent living and community integration.

Qualifications

• Bachelor’s degree in social work, psychology, or a related field.
• Strong written and verbal communication skills.
• Bilingualism is preferred.
• Exceptional interpersonal skills with compassion and firmness.
• Confidentiality must be maintained.

Responsibilities

• Promptly conducting clients’ initial intake assessments and addressing critical needs.
• Completing bio-psycho-social assessments within 30 days of intake.
• Developing Independent Living Plans (ILPs) with other providers.
• Documenting and maintaining client records with confidentiality.
• Connecting clients to housing, employment, and supportive services.
• Meeting clients regularly to assess progress and adjust plans as needed.
• Coordinating referrals to housing, DHS or HRA appointments, and community resources.
