CAMBA Inc. is hiring a Case Manager in New York City to conduct initial intake, assess client needs, and coordinate access to housing and supportive services.

You will maintain thorough client files, update evaluations, and collaborate with a multidisciplinary team to help clients achieve stable housing and improved well-being. The role requires a Bachelor’s degree (or equivalent experience) and bilingual English/Spanish abilities.

Qualifikationen

• Bachelor’s degree (e.g., B.A., B.S.W.) or four years of relevant experience.
• Associate’s degree with two years of equivalent experience.
• Bi-lingual English & Spanish preferred.

Aufgaben

• Conduct initial intake and assessment of clients’ needs and periodic re-assessments.
• Create and maintain client electronic and paper files.
• Prepare initial psychosocial evaluation and update according to regulations.
• Obtain and maintain relevant documentation (birth certificate, proof of income, medical documents, PHI releases, etc.).
• Collaborate with clients to revise independent living plans with short- and long-term goals.
• Assist clients in attaining goals by identifying community resources and referring to CAMBA and external services.
• Support clients in overcoming barriers to goal attainment.
• Coordinate with other social services staff and provide client advocacy for housing stability.
• Schedule appointments and escort clients to educational, medical, or social service meetings.
• Document encounters in DHS CARES Database and update paper files for progress reports.
