Job summary

Resources for Human Development (RHD) is seeking a Case Manager in Medford, MA to support individuals in behavioral health, recovery, and independent living goals. You will work with a multidisciplinary team to connect participants to services, resources, and supports in the community.

The role requires a high school diploma (or equivalent) plus experience in behavioral health or IDD services, valid driver''s license, and strong communication.

Qualifications

• Must have a High School Diploma or equivalent.
• Minimum two years of experience supporting individuals in behavioral health, addiction recovery, or IDD services.
• Valid driver''s license and reliable vehicle with required insurance.
• Intermediate computer proficiency including Microsoft Word, Excel, and data entry systems.
• Ability to pass required background screenings including FBI clearance if applicable.
• Strong communication, organizational, and problem-solving skills.

Responsibilities

• Collaborate with multidisciplinary teams to provide holistic, person-centered services.
• Assist in development, implementation, and review of recovery and treatment plans.
• Connect participants with community resources, benefits, and support services.
• Build and maintain relationships with community agencies and service providers.
• Deliver community-based services focused on recovery, education, and community integration.
• Transport participants to appointments and services as needed.
