Job summary

Resources for Human Development (RHD) is seeking a compassionate Case Manager in Massachusetts to help individuals access services, achieve recovery goals, and build more independent lives. The OTL team connects people to essential supports and resources to thrive.

As a full-time Case Manager, you will collaborate with multidisciplinary teams, develop recovery plans, and advocate for participant needs in office and community settings. DailyPay and comprehensive benefits are included.

Qualifications

• High School Diploma or equivalent.
• Minimum two (2) years of experience supporting individuals in behavioral health, addiction recovery, or IDD services.
• Valid driver's license and reliable vehicle with required insurance.
• Intermediate computer proficiency, including Microsoft Word, Excel, and data entry systems.
• Ability to pass required background screenings, including FBI clearance if applicable.
• Strong communication, organizational, and problem-solving skills.
• Ability to multitask and work effectively within a team.

Responsibilities

• Collaborate with multidisciplinary teams to provide holistic, person-centered services.
• Assist in the development, implementation, and review of recovery and treatment plans.
• Connect participants with community resources, benefits, and support services.
• Build and maintain relationships with community agencies and service providers.
• Deliver community-based services focused on recovery, education, and community integration.
• Transport participants to appointments and services as needed.
• Advocate for participants to access necessary medical, behavioral health, housing, and social supports.
• Complete assessments, service documentation, and reports in compliance with regulatory requirements.
• Monitor progress toward goals and support plan implementation.
• Maintain accurate records in accordance with organizational, funder, and regulatory standards.
• Identify service gaps and recommend solutions to improve participant outcomes.
