Job summary

RHD seeks a dedicated Case Manager to coordinate services and support individualized recovery goals for participants in behavioral health services. You will collaborate with clinical staff, community providers, and participants to implement service plans and advocate for access to needed resources.

The role emphasizes holistic, team-based care, compassionate service, and advocacy within a mission-driven organization. Local travel to community sites is required.

Qualifications

• Bachelor’s degree in human services or related field.
• At least two years of general work experience providing support to individuals receiving behavioral health services.
• Intermediate computer skills.
• Valid driver’s license.
• Ability to travel throughout the program’s service-delivery area.
• Ability to transport program participants to appointments.

Responsibilities

• Participate as a member of a multidisciplinary team providing holistic, values-based services.
• Assist with development and review of individualized recovery plans in collaboration with the program participant, clinical and medical staff, and other stakeholders identified with the participant.
• Help determine the resources and services needed to support each participant’s individualized goals.
• Advocate for program participants to help ensure access to appropriate services and supports.
• Identify program needs and service gaps and recommend strategies for addressing them.
• Develop and maintain productive relationships with community agencies.
• Connect program participants with available community resources and services.
• Provide community-based services to program participants.
• Educate participants about available services, supports, and resources.
• Assist participants with making meaningful connections in their communities.
• Support participants in working toward the goals identified in their recovery plans.
• Transport program participants to appointments as required.
• Complete assessments, service documentation, and other records accurately and within required timeframes.
• Maintain documentation in accordance with requirements and standards.
• Participate in program planning, coordination, and service-review activities.
• Perform other job-related duties as assigned.
