Job summary

Resources for Human Development (RHD) is seeking a Case Manager to support individuals in Camden, DE through recovery-oriented, person-centered services. You will assess needs, coordinate services, advocate for access to resources, and help clients achieve stability, wellness, and independence in collaboration with a multidisciplinary team.

The role involves substantial community-based work, on-call coverage, documentation, and adherence to HIPAA and funding requirements to ensure quality

Qualifications

Bachelor's degree in Human Services or related field required.
Valid driver's license and reliable transportation required.
Ability to pass federal and state criminal background checks.
Minimum of two (2) years of experience in human services, behavioral health, or related field.

Responsibilities

Assess participant strengths, needs, goals, and barriers to determine appropriate services and resources.
Assist in development, implementation, and review of individualized recovery plans.
Coordinate services with participants, supports, and stakeholders to achieve recovery goals.
Maintain accurate documentation and ensure compliance with policies, funding, and HIPAA requirements.
