Job summary

RHD is seeking a dedicated Case Manager to identify resources and support individualized service plans for program participants in the Philadelphia, PA area. The role emphasizes holistic, team-based care, advocacy, and connections to community services.

You will coordinate services with clinical teams, providers, and stakeholders to meet recovery goals, document progress, and ensure access to needed supports in community settings.

Qualifications

• Bachelor’s degree in human services or a related field.
• At least two years of general work experience providing support to individuals in behavioral health settings.
• Intermediate computer skills.
• Valid driver’s license.
• Ability to travel throughout the program’s service-delivery area.
• Ability to transport program participants to appointments as required.

Responsibilities

• Participate as a member of a multidisciplinary team providing holistic, values-based services.
• Assist with development and review of individualized recovery plans.
• Help determine resources and services to support participants’ goals.
• Advocate for program participants to access appropriate services.
• Identify program needs and service gaps and propose strategies.
• Develop and maintain relationships with community agencies.
• Connect participants with available community resources.
