Job summary

Volunteers of America Los Angeles is seeking a Case Manager Generalist to provide case management services to clients with low acuity, coordinating services and care, maintaining a caseload, and serving as the primary point of contact.

The role emphasizes on-site work, comprehensive assessments, developing case plans, coordinating care, advocacy, and connecting participants to resources; requires fingerprint clearance, HS/GED, and 1 year experience or a BA in Social Services.

Qualifications

• Experience in case management or similar social services role desired.
• Fingerprint clearance/background check required.
• VOALA Case Management training within 6 months of hire.

Responsibilities

• Services for this position must be provided on-site.
• Assessments and planning to meet client needs and goals.
• Coordinate care and case management as part of a team.
• Advocacy for participants before other agencies and community settings.
• Engagement and outreach to connect participants to services.

Education
