Job summary

VOALA is seeking a Case Manager Generalist to provide case management to clients with low acuity and to coordinate services and link clients to resources on-site in Los Angeles.

The role involves conducting assessments, developing case plans, coordinating care with a multidisciplinary team, and advocating for clients before community and social service agencies. Mandated reporter status and fingerprint clearance are required; HS/GED and relevant training are needed.

Qualifications

• Fingerprint clearance and background check required.
• Ability to work with challenging, potentially aggressive clients.
• 1-year general experience in case management or a Bachelor’s Degree in Social Services in lieu of experience.

Responsibilities

• Conduct comprehensive screenings and assessments to collect client information.
• Develop case plans and coordinate service linkages to meet client needs.
• Advocate for clients before agencies and coordinate care with the service team.
