Job summary

Volunteers of America Los Angeles is seeking a Case Manager Generalist to coordinate services for clients with low acuity. You will handle a caseload, perform comprehensive screenings, and connect clients to needed resources on-site every day.

Responsibilities include developing individualized plans, facilitating service linkages, and advocating with external agencies to meet goals. VOALA emphasizes serving vulnerable populations with extensive benefits and professional development opportunities.

Qualifications

• HS/GED. Completion of VOALA Case Management training within 6 months of hire.
• Minimum 1-year general experience in case management or similar social services role or Bachelor’s Degree in Social Services related field in lieu of experience
• Preferred: Bachelor’s Degree (Social Work, Social Services, public administration, public health or related field) and HMIS navigation experience.

Responsibilities

• Conducts screenings and assessments to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information.
• Assess barriers and develop a case plan to meet short- and long-term needs.
• Coordinate care and case management with team and connect participants to services.
• Advocate for participants before agencies and community settings.
