Job summary

PAM Health Rehabilitation Hospital of Overland Park seeks a Case Manager to coordinate interdisciplinary care for a designated patient population within inpatient settings. You will review admission records to ensure appropriate resources and level of care, and identify discharge needs with an effective discharge plan.

You will work with the interdisciplinary team to achieve desired financial and quality outcomes and maintain payor relationships, contributing to high-quality patient care.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines of the designated case manager may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care for a designated patient population across inpatient settings.
• Review inpatient admission records for proper utilization of hospital resources and determination of admission level of care.
• Assess discharge needs and coordinate appropriate discharge plan.
• Work collaboratively with the interdisciplinary team to facilitate achievement of financial and quality outcomes.
• Maintain and enhance payor relationships.
