Job summary

PAM Health is seeking a Case Manager to coordinate interdisciplinary care for designated patient populations within our hospital network. You will review admission records to ensure appropriate utilization and work with the team to facilitate effective discharge planning.

Responsibilities include assessing discharge needs, coordinating plans, and maintaining strong payor relationships to achieve quality and financial goals.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include OT, PT, RN, RT, SLP, or licensed social worker.
• Current BLS certification required.

Responsibilities

• Coordinate interdisciplinary care for designated patient populations.
• Review inpatient admission records to ensure appropriate utilization of hospital resources.
• Assess discharge needs and coordinate suitable discharge planning.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
