Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a full-time Case Manager to coordinate interdisciplinary care for an identified patient population. You will review admission records to ensure appropriate resource use and determine level of care, while planning discharge needs with the team.

The role requires 3–5 years of inpatient experience (acute/IRF/LTACH) and active BLS certification.

Qualifications

• Licensure in the state where the hospital resides is preferred; case manager disciplines may include OT, PT, RN, RT, SLP, or social worker; Current BLS certification required.
• 3–5 years inpatient experience, preferably in an acute, IRF, or LTACH setting.

Responsibilities

• The Case Manager coordinates care across disciplines for designated patients and reviews inpatient admission records for appropriate level of care.
• Assesses discharge needs and coordinates the discharge plan.
• Maintains payor relationships and collaborates with the interdisciplinary team to achieve financial and quality outcomes.
