Job summary

PAM Health, LLC is seeking a Case Manager to coordinate care for a designated patient population within a hospital setting. You will review inpatient records to ensure appropriate admission and level of care, and work with the interdisciplinary team to optimize outcomes and payor relations.

Responsibilities include coordinating discharge planning and identifying patient needs, supported by a strong background in case management and licensed professionals in relevant disciplines.

Qualifications

• Licensure in the state where PAM Health hospital resides.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care for a designated patient population and review inpatient records to ensure appropriate admission and level of care.
• Identify discharge needs and coordinate discharge planning, working with the interdisciplinary team to achieve desired outcomes and maintain payor relationships.
