Job summary

PAM Health Rehabilitation Hospital of Miamisburg is seeking a Case Manager to coordinate interdisciplinary care for a designated patient population. You will review admission records to ensure appropriate resource use and determine admission level of care, while coordinating discharge planning to meet patient needs.

You will work with the interdisciplinary team to achieve financial and quality outcomes and maintain payor relationships, supporting our mission to provide high-quality post-acute

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Current BLS certification required.
• Three to five years of inpatient experience

Responsibilities

• Coordinate the interdisciplinary care of a designated patient population.
• Review inpatient admission records to verify appropriate level of care.
• Assess discharge needs and coordinate discharge planning.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
