You will review inpatient admission records to ensure proper utilization of resources and determine admission level of care, while coordinating discharge planning with the care team.

The role emphasizes collaboration with OT, PT, RN, RRT, SLP, and social work, maintaining payor relationships, and supporting quality

Qualifications

• Education and Training: Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include OT, PT, RN, RRT, SLP, or LSW.
• Current BLS certification required.

Responsibilities

• The Case Manager is responsible for the interdisciplinary coordination of care for a designated patient population.
• Performs reviews of inpatient admission records to ensure proper utilization of hospital resources and determination of admission level of care.
• Assesses discharge needs and coordinates appropriate discharge plan with the interdisciplinary team.
• Maintains and enhances payor relationships.
