Job summary

PAM Health Rehabilitation Hospital of Waterbury is seeking a Case Manager to coordinate interdisciplinary care for a designated patient population within the hospital setting.

The Case Manager will review inpatient admission records, assess discharge needs, and coordinate discharge planning while collaborating with the interdisciplinary team to achieve financial and quality outcomes, and maintain payor relationships.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
• Current BLS certification required.

Responsibilities

• Coordinate interdisciplinary care for designated patient population.
• Review inpatient admission records to ensure appropriate level of care.
• Assess discharge needs and coordinate discharge planning.
• Collaborate with the care team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
