Job summary

PAM Health Specialty and Rehabilitation Hospitals of Miamisburg is seeking a dedicated Case Manager to coordinate care for an identified patient population within our hospital. You will review admission records, determine appropriate care levels, and lead discharge planning in collaboration with an interdisciplinary team.

Responsibilities include coordinating with case management, payor relations, and hospital staff to ensure quality outcomes and efficient utilization of resources.

Qualifications

• Licensure in the state where the hospital resides preferred.
• Current BLS certification required.
• Three to five years of inpatient experience preferred in acute, IRF, or LTACH settings.

Responsibilities

• Coordinate care across disciplines for designated patient populations.
• Review inpatient admission records to ensure appropriate level of care and utilization.
• Assess discharge needs and coordinate discharge planning.
• Collaborate with interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
