Job summary

PAM Health, LLC is seeking a Case Manager to coordinate interdisciplinary care for designated patient populations. You will review inpatient records to ensure appropriate resource use and determine admission levels, while coordinating discharge planning and collaborating with the care team to reach financial and quality targets.

You will also maintain and enhance relationships with payors, supporting optimal patient outcomes and efficient service delivery across the facility.

Qualifications

• State licensure preferred; professional disciplines may include OT, PT, RN, RRT, SLP, or social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care across disciplines to ensure proper resources and discharge planning.
• Maintain payor relationships and support achievement of financial and quality goals.
