Job summary

PAM Health is seeking a Case Manager to coordinate interdisciplinary care for designated patients within a hospital setting. The role focuses on reviewing inpatient records to ensure appropriate utilization and facilitating discharge planning in collaboration with the care team.

The ideal candidate will have state licensure, relevant professional background (OT, PT, RN, RT, SLP, or social work), and current BLS certification, with 3–5 years inpatient experience preferred in acute or LTACH

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care for designated patient population across inpatient settings.
• Review inpatient admission records to ensure appropriate resource use and level of care.
• Identify discharge needs and coordinate discharge planning with the team.
• Maintain and enhance payor relationships and collaborate with interdisciplinary team.
