CVS Health Corporation is seeking a Case Management Coordinator to support members by coordinating appointments, identifying benefits and educating individuals through care management tools and resources.

The role requires 40 hours weekly, 50-75% travel in DuPage, Kane, DeKalb, Kendall counties and surrounding areas, with standard business hours. Telework available with travel to meet member needs and program goals.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willing to travel 50-75% in IL counties; reliable transportation.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members' needs and determine case resolution based on benefits.
• Identify high risk factors and coordinate appropriate referrals.
• Coordinate and monitor care plan activities and progress.
• Collaborate with clinicians to improve quality of care and overcome barriers.
• Escalate quality of care issues through proper channels.
• Engage with members to support informed healthcare decisions.
• Document care and ensure regulatory compliance.
