Responsibilities

• Evaluate members' needs/eligibility and recommend case resolution approaches.
• Identify high-risk factors and coordinate appropriate referrals to clinical case management or crisis intervention.
• Coordinate and monitor assigned care plan activities and progress.
• Consult with case managers, supervisors and Medical Directors to address barriers and improve outcomes.
• Ensure compliance with regulatory/accreditation guidelines and company policies.
