Responsibilities

• Evaluate members' needs/eligibility and inform care plan strategy.
• Identify high risk factors and appropriate referrals to clinical case management or crisis intervention.
• Coordinate and monitor care plan activities and progress.
• Provide coaching and support to empower members to engage with providers and manage health decisions.
