Job summary

Encompass Health is seeking a Case Management Assistant to streamline workflow, improve communications, and support the Case Management department across our inpatient rehabilitation network.

You will coordinate team conference meetings, gather and maintain referrals, admissions, orders, and schedules, and assist with discharge activities, regulatory readiness, and data collection. Strong MS Office skills and clear communication are essential.

Qualifications

• High school diploma or equivalent preferred.
• Proficiency in typing with speed and accuracy.
• Knowledge of medical terminology preferred.
• Experience with insurance/payor systems preferred.

Responsibilities

• Coordinate and facilitate Team Conference meetings.
• Gather, maintain, and action on Case Management referrals, admissions, orders, and schedules.
• Communicate daily Case Management assignment updates.
• Support pre- and post-discharge activities to inform patients and families.
• Assist in regulatory readiness and Joint Commission survey preparation.
• Collect Quality Improvement data and department metrics.
