Job summary

Encompass Health Corporation in Illinois is seeking a Case Management Assistant to streamline workflow, facilitate communications, and support the Case Management team in delivering patient-centered care.

You will coordinate referrals, admissions, orders, and discharge schedules with care teams, gather data for quality improvement, and assist in regulatory and Joint Commission readiness.

Qualifications

• High school diploma or equivalent preferred.
• Proficiency in Microsoft Office products.
• Strong typing skills with speed and accuracy.
• Knowledge of medical terminology (preferred).
• Experience with insurance/payor systems (preferred).
• Good visual acuity and communication skills.
• Flexibility to work weekdays/weekends, evenings, or night shifts if necessary.
• Availability during religious/legal holidays as scheduled.

Responsibilities

• Coordinate and facilitate Case Management activities and communications.
• Maintain Case Management documentation including referrals, admissions, orders & schedules.
• Disseminate daily Case Management assignment updates to the team.
• Assist with pre- and post-discharge activities for patients, families, and clinicians.
• Assist in collecting Quality Improvement data and performance metrics.
• Support regulatory and survey preparedness activities.
