Job summary

Encompass Health Corporation is seeking a Case Management Assistant to streamline workflows, coordinate communications, and support the Case Management team. The role emphasizes collaboration with clinicians, patients, and families to ensure smooth discharge planning and care continuity.

The position offers opportunities to engage with community resources, participate in performance improvement activities, and contribute to a patient-centered care mission across a national inpatient

Qualifications

• High school diploma or equivalent preferred.
• Proficiency in Microsoft Office products.
• Strong typing skills with speed and accuracy.
• Knowledge of medical terminology (preferred).
• Experience with insurance/payor systems (preferred).
• Good visual acuity and communication skills.
• Flexibility to work weekdays/weekends, evenings, or night shifts if necessary.
• Availability during religious/legal holidays as scheduled.

Responsibilities

• Coordinate and facilitate Team Conference meetings.
• Gather and maintain Case Management documentation (referrals, admissions, orders, schedules).
• Communicate daily Case Management Assignment updates.
• Complete pre- and post-discharge activities for patients and families.
• Collect Quality Improvement and department Performance data.
• Assist with collection of Key Care Indicators and clinical outcomes data.
• Support regulatory and Joint Commission survey preparedness activities.
