Encompass Health in Concord, NH is seeking a dedicated Case Management Assistant to support the Case Management team by streamlining workflows, coordinating meetings, and handling documentation for referrals, admissions, orders, and schedules.

You will partner with clinicians and patients to ensure timely communication, assist with pre- and post-discharge activities, and contribute to data collection for quality improvement in a friendly, inclusive workplace.

Qualifikationen

• High school diploma or equivalent preferred.
• Proficiency in Microsoft Office products.
• Strong typing skills with speed and accuracy.
• Knowledge of medical terminology (preferred).
• Experience with insurance/payor systems (preferred).
• Good visual acuity and communication skills.
• Flexibility to work weekdays/weekends, evenings, or night shifts if necessary.
• Availability during religious/legal holidays as scheduled.

Aufgaben

• Coordinate and facilitate Team Conference meetings.
• Gather and maintain Case Management referrals, admissions, orders, and schedules.
• Coordinate communication of daily Case Management assignment updates.
• Complete pre- and post-discharge activities to inform patients, families, and clinicians.
• Collect Quality Improvement and department performance data.
• Assist with Key Care Indicators and clinical outcomes data.
• Assist in regulatory and Joint Commission survey preparedness activities.
