Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of the discipline (RN, SW, OT, PT, ST, Rehab Counseling).
• Active license required if licensure is mandated by state.
• CCM or ACM certification required or obtained within two years of start.
• Bachelor's degree minimum for all other licensed/certified health professionals; graduate degree preferred.

Responsibilities

• Collaborate with interdisciplinary team to guide treatment plans based on patient needs and preferences.
• Coordinate discharge planning and contingency plans with the interdisciplinary team.
• Plan and execute patient discharge experiences and post-discharge follow-up as needed.
• Monitor patient experience across quality, timeliness, service appropriateness, and payor expectations.
• Facilitate weekly team conferences and update treatment plan changes.
• Complete case management addenda and required documentation.
• Maintain knowledge of regulations, standards, and department policies.
