Job summary

Encompass Health is seeking a Case Management Assistant to streamline workflows, facilitate communications, and enhance our Case Management department’s efficiency. You will engage with local resources and support care coordination for patients across services.

The role emphasizes collaboration, data collection, and regulatory readiness while supporting discharge processes and patient care planning in a national inpatient rehabilitation setting.

Qualifications

• High school diploma or equivalent required.
• Proficiency in Microsoft Office products.
• Strong typing skills with speed and accuracy.
• Knowledge of medical terminology preferred.
• Experience with insurance/payor systems preferred.
• Good visual acuity and communication skills.
• Flexibility to work varied shifts and holidays as needed.

Responsibilities

• Coordinate and facilitate Team Conference meetings.
• Gather, act on, and maintain Case Management documentation (referrals, admissions, orders, schedules).
• Communicate daily Case Management Assignment updates.
• Complete pre- and post-discharge activities to prepare patients, families, and clinicians.
• Gather Quality Improvement and department performance data.
• Assist in collection of Key Care Indicators and clinical outcomes data.
• Support regulatory and Joint Commission survey preparedness activities.
