Job summary

HighPoint Hospice, part of the LHC Group within Optum family, seeks a Home and Community Based Services Aide to support clients at home. You will assist with personal care, daily living activities, meals, and accompany clients to appointments, while documenting care and maintaining a safe home environment.

Requirements include a valid driver’s license, 75 hours HHA training or equivalent, CPR certification or plan to obtain, and 6+ months of home care experience. Competitive benefits offered.

Qualifications

• Current driver’s license and reliable transportation required.
• CPR certification or ability to obtain within 90 days of hire.
• 6+ months of home care experience preferred.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Ability to work in various home settings regardless of client background.
