Job summary

At Home Hospice, a part of LHC Group, is seeking a Home and Community Based Services Aide to support clients at home with personal care and daily tasks. You will monitor client condition, ensure safety, and coordinate with families and care teams. A valid driver’s license and prior home health experience are required, with benefits and competitive pay.

Qualifications

Current driver’s license, vehicle insurance, and reliable transportation.
Minimum 1 year full-time direct client care in hospital or nursing facility, OR equivalent home health care experience within last 5 years.
Must NOT be listed on the Employee Misconduct Registry and DHS Nurse Aide Registry status must be clear.

Responsibilities

Provide personal care and assist with daily living activities.
Support household tasks, meal preparation, and accompany clients to appointments or errands.
Monitor and document client condition, vital signs, and incidents; maintain confidentiality and EVV use.
Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition.
