Job summary

Heart of Hospice, part of LHC Group and a UnitedHealth Group family, seeks a Home and Community Based Services Aide to support clients in their homes, enabling independence and comfort. You will provide personal care, household support, and companionship in a caring environment.

You will document vitals and incidents, ensure safety, and comply with state requirements, including Louisiana HHA standards. Competitive hourly pay and benefits offered to eligible staff.

Qualifications

• Current CNA certification or completion of a CNA training program.
• Current First Aid Certification.
• State-specific: LA Home Health Aide certification or Medicare provider requirements.

Responsibilities

• Provide personal care and assist with daily living activities (bathing, grooming, dressing, ambulation, medication reminders).
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Adhere to non-discrimination practices in care regardless of client background.
