Job summary

Elite Hospice, part of the LHC Group and Optum family, seeks a Hospice Home and Community Based Services Aide to support individuals at home. Responsibilities include personal care, daily living activities, mobility assistance, and accompanying clients to appointments.

Must have a valid driver’s license and reliable transportation, with TX-specific care experience or training requirements addressed within 90 days.

Qualifications

• Current driver’s license, vehicle insurance, and reliable transportation.
• CPR certification (or able to complete within 90 days).
• 6+ months of home care experience (TX-specific requirement).

Responsibilities

• Provide personal care and assist with daily living activities (bathing, grooming, dressing, ambulation, medication reminders).
• Support household tasks, meal preparation, and accompany clients to appointments or errands.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
