Heart n Home Hospice is seeking a Home and Community Based Services Aide to help clients remain in their own homes with personal care, daily living tasks, and companionship. You will assist with hygiene, meals, errands, and transport as needed, while documenting care and staying compliant with privacy standards.

Ideal candidates have a driver's license, reliable transportation, and perhaps CPR certification. Flexible hours and independence are valued in this role.

Qualifications

• Current driver's license, vehicle insurance, and reliable transportation or access to public transportation.
• CPR certification preferred or ability to obtain within 90 days.
• 6+ months of home care experience.
• Ability to work flexible hours and independently.

Responsabilités

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
