Responsibilities

• Screen patients' medical records and document clinical information
• Facilitate referrals to community agencies for transfer to ECFs or home care
• Collaborate with Social Worker/Case Manager to place patients based on needs and insurance
• Communicate with external agencies to arrange transfer and transportation
• Investigate insurance coverage for long-term or outpatient care as needed
• Build relationships with community agencies to promote expedient transfers
• Maintain awareness of state and federal regulations affecting discharge planning
• Provide updates to Social Work and Case Management staff on policy changes
• Assist in orientation of new staff regarding ECF and transportation processes
• Compile departmental statistics for monthly/annual reports and studies
• Develop referral tools to enhance continuity of care
• Independently organize priorities and maintain professional rapport
• Maintain patient confidentiality and safety at all times
• Align performance with Mission, Vision and Values of St. Joseph Mercy Health System
