Job summary

Trinity Health At Home – Michigan seeks a Clinical Home Care Coordinator (RN, LPN, Therapist or Social Worker) to maintain daily contact with the care team, coordinate hospital-to-home transitions, and arrange appropriate homecare services. You will review medical records, obtain physician orders, and document initial homecare assessments to ensure timely, compliant care planning.

Qualifications

• Well-developed oral and written communication skills.
• Must have a minimum of two years' experience in an acute care setting, customer service, sales/marketing, or relevant experience.
• Must have a current driver's license and reliable transportation.

Responsibilities

• Maintains daily contact with case managers/discharge planners and units to ensure timely follow-up for referred patients.
• Coordinates care for clients going home from the hospital who select agency services.
• Reviews hospital records and gathers information for transmission as needed.
• Obtains physician orders and documents initial assessments of homecare needs.
