Responsibilities

• Screen patients' medical records and document discharge plans using continuity of care tools.
• Facilitate referrals to community agencies and arrange transfers to ECFs or home care.
• Collaborate with Social Worker/Case Manager to place patients at appropriate facilities.
• Communicate with ECFs about patient placement considering desires, clinical needs, and insurance.
• Investigate insurance coverage for long-term or outpatient care.
• Develop relationships with community agencies to promote expedient transfers.
• Maintain awareness of regulations and reimbursement issues related to continuity of care.
• Prepare monthly/annual reports on patient transfers and related studies.
• Participate in orientation of new staff regarding ECF/transport processes.
• Maintain confidentiality of patient and hospital information.
