Job summary

Trinity Health Grand Rapids is seeking an experienced Registered Nurse to join our Transfer Center as a Transfer Center Coordinator. You will serve as the centralized point of contact for EMS patient transfers, ensuring safe and timely transitions of care across the region.

The role involves coordinating interfacility transfers, triage decisions, and collaborating with dispatchers and clinical teams to optimize patient flow and bed management.

Qualifications

• Bachelor’s degree in nursing or related health field.
• Current Michigan RN license.
• Active BLS certification.
• Minimum of 5 years of progressive acute care clinical experience.
• Strong communication, coordination, and problem-solving skills.

Responsibilities

• Coordinate and manage EMS interfacility transfers for inpatient and emergency spaces.
• Collaborate with Trinity Health EMS dispatchers to triage and prioritize transfers.
• Assist in prioritization of facility discharge transports, including discharges to skilled nursing facilities, assisted livings, and inpatient psychiatric care.
• Collaborate with Clinical Resource Coordinators on discharge transport needs to streamline patient flow and bed management throughout the facility.
• Collect, document, and communicate critical clinical and logistical patient information.
• Support ED-to-ED transfers and continuity of care.
• Identify barriers to patient flow and escape concerns appropriately.
• Participate in quality and process improvement initiatives (PDCA, LEAN).
• Deliver outstanding customer service aligned with Excellence in Care Experience principles.

Education

Bachelor’s degree in nursing or related health field

Current Michigan RN license

Active BLS certification

Excellent communication and problem-solving skills
