Job summary

Trillium Health Resources seeks a Care Transitions Care Manager to provide comprehensive care coordination for individuals transitioning between care settings, ensuring continuity and stability. You will assess needs, develop care plans, and coordinate across providers and community resources for long-term outcomes.

This role serves My Choice program participants with IDD in intermediate care facilities or State Developmental Centers, requiring licensure or RN with MH/DD/SA experience and travel

Qualifications

• Must meet QP status as defined by NC regulations.
• RN licensed in NC or NC-licensed Professional with MH/DD/SA experience.
• Master's degree in human services with 1 year MH/DD/SA experience.
• Bachelor's degree in human services with 2 years MH/DD/SA experience.
• Bachelor's in non-human services with 4 years MH/DD/SA experience.
• Two years LTSS/HCBS coordination experience preferred (LTSS).
• Valid driver’s license required.
• Must reside in Anson, Richmond, Moore, Scotland, Hoke or Robeson counties and travel as needed.

Responsibilities

• Manage ongoing care for assigned individuals across services and settings.
• Coordinate services across providers and community resources.
• Educate individuals on available services and resources.
• Link individuals to community resources and support community integration.

Education

Licensed RN or Licensed by NC Board of Nursing

Masters in Human Services field

Bachelor in Human Services or related field

Two years MH/DD/SA experience (Bachelor holder)

Four years MH/DD/SA experience (Bachelor level with 4 years)

Valid driver's license
