Job summary

Qualifications

• Minimum of one year of experience in home health or hospital-based case management.
• Current and active licensure in the state of practice as an RN, LPN, SW or PT.

Responsibilities

• Achieve monthly production goals and Medicare-certified admission targets for assigned locations.
• Evaluate patients and physician orders for home care eligibility.
• Conduct face-to-face patient transitions to provide agency education and identify the primary care physician responsible for the plan of care.
• Coordinate transfer orders and ancillary services (e.g., DME, infusion) after patient acceptance.
• Educate patients on post-discharge physician appointments, prescriptions, and medication regimens.
• Act as liaison between the agency and healthcare providers for referred and transferred patients.
• Maintain patient confidentiality and ensure timely communication with referral sources.
